Job summary

UnitedHealth Group, a leading health company, is seeking a Registered Nurse Case Manager for our hospice program. You will deliver the Plan of Care for each patient, collaborate with a multidisciplinary team, and ensure high-quality, compassionate care in home settings.

The role requires an unrestricted RN license in MS, reliable transportation, and the ability to work within a caring, diverse patient population. Strong clinical skills and commitment to patient well-being are essential.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• MS state: 1+ years full-time RN experience; substitution: 3 years LPN for RN experience.
• CPR certification or ability to complete within 90 days of hire (preferred).

Responsibilities

• Makes initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies the patient/family needs and re-assesses >= every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response.
• Collaborates with patient/family, attending physician, and IDG members.
• Instructs patient/family in self-care when appropriate.
• Maintains accurate clinical notes on patient condition.
• Provide care in diverse home settings regardless of demographics.
