Job summary

At Home Hospice, part of LHC Group under the Optum family, seeks a Registered Nurse Case Manager to deliver and continuously improve the Plan of Care for hospice patients. You will collaborate with multiple professionals to implement treatment strategies supporting physical, psychosocial, emotional, and spiritual well-being.

The role requires an unrestricted RN license and valid driver’s license; CPR is preferred and a vehicle is needed for patient home visits.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current driver’s license and access to a dependable vehicle or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within 48 hours of referral.
• Identifies patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care; documents goals, interventions and responses.
• Collaborates with patient/family, attending physician and IDG members daily.
• Instructs and supervises patient/family in self-care techniques when appropriate.
