Job summary

Heart n Home Hospice, part of the Optum family, seeks a Hospice Registered Nurse Case Manager to deliver the Plan of Care for each patient and ensure quality nursing care in a home hospice setting.

The role requires an active RN license in the practicing state, reliable transportation, CPR eligibility, and 1+ year of clinical experience. A $5,000 sign-on bonus is offered for external candidates. Benefits include comprehensive packages and 401k contributions.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.
• 1+ years of clinical experience

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within 48 hours of referral.
• Identifies the patient's/family's physical, psychosocial, and environmental needs and reassesses as needed at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes regarding the patient's condition.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with the patient/family, attending physician and other IDG members in daily patient/family care.
