Job summary

Journey Hospice is seeking a full-time RN Case Manager to serve clients in Adams and Franklin Counties. The RN will oversee a primary patient caseload, delivering skilled nursing care and coordinating with the IDT to implement the plan of care.

Responsibilities include initial and ongoing nursing assessments, teaching and supervising patients and families, and ensuring care is responsive to needs. A PA RN license and valid driver's license are required.

Qualifications

• Graduate of an accredited registered nursing program.
• PA RN license in good standing.
• Bachelor’s degree preferred.

Responsibilities

• Oversee a primary hospice patient caseload and provide skilled nursing care as prescribed.
• Perform initial and comprehensive nursing assessments and develop the IDT plan of care.
• Educate, supervise, and counsel patients, families, and staff on nursing care and needs.
