Job summary

Journey Hospice is seeking a Full-Time RN Case Manager to serve clients in Adams and Franklin Counties. The role oversees a primary caseload and delivers skilled nursing care within the Hospice program.

The RN will perform assessments, develop the interdisciplinary plan of care, and collaborate with family members and the IDT to meet patient goals. A PA RN license, good driving record, and compassionate approach are required.

Qualifications

• Must maintain a professional attitude and demeanor and be able to communicate effectively with patients, caregivers, colleagues, supervisors, and all others.
• Must demonstrate optimism and willingness to work constructively with the health care team.
• Interacts with all staff in a positive fashion supporting the mission and vision of Journey Hospice.
• Minimum of one year general nursing; hospice, home health, medical, surgical or critical care experience preferred.

Responsibilities

• Provide initial and comprehensive nursing assessment for Hospice patients.
• Develop and reassess the Hospice plan of care in collaboration with the IDT.
• Implement and revise the plan to meet patient nursing needs.
• Educate, supervise, and counsel patients, families, and staff on nursing care and needs.
• Supervise Hospice Aides/Homemakers and coordinate with other IDT members.
