Job summary

Journey Hospice in Pennsylvania is seeking a Full-Time RN Case Manager to serve our Hospice patients in Adams and Franklin Counties. You will oversee a primary caseload and deliver skilled nursing care as prescribed, coordinating with the Interdisciplinary Team.

The role includes initial and ongoing assessments, care planning, teaching families, supervising Hospice Aides, and collaboration with IDT. PA RN licensure and a BSN are preferred; valid driver’s license required.
