Job summary

UP Health System Hospice, part of LHC Group within the Optum family, seeks an RN Case Manager to manage the Plan of Care for hospice patients. You will collaborate with a care team to implement strategies addressing physical, psychosocial, emotional, and spiritual needs.

Responsibilities include initial eligibility assessments within 48 hours, developing and updating the Plan of Care with the IDG, and maintaining accurate clinical notes while ensuring quality and compassionate care for diverse

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• Preferred: 1+ years of clinical experience; CPR certification or ability to obtain within 90 days.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify and reassess patient/family needs at least every 15 days; initiate and coordinate care plan.
• Document problems, goals, interventions, and patient/family response to care.
