Job summary

UnitedHealth Group is hiring a Registered Nurse Case Manager for UP Health System Hospice, part of LHC Group. You will deliver and adjust the Plan of Care for hospice patients, collaborating with a team to promote physical, psychosocial, emotional, and spiritual well-being in home settings across Michigan.

You will evaluate patients within 48 hours of referral, coordinate care, and ensure documentation and communication with families and the care team.

Qualifications

• Current CPR certification or ability to obtain within 90 days of hire
• 1+ years of clinical experience
• Current and unrestricted RN licensure in the state of practice
• Valid driver's license, vehicle insurance, and reliable vehicle or access to public transportation

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral
• Identifies physical, psychosocial, and environmental needs and re-assesses at least every 15 days
• Initiates and coordinates the plan of care
• Documents problems, goals, interventions, and patient/family response to hospice care
• Collaborates with patient/family, attending physician, and IDG members in daily care
• Instructs and supervises patient/family in self-care techniques when appropriate
• Maintains accurate clinical notes of patient condition
• Functions in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition
