Job summary

Elite Hospice, part of LHC Group and a member of the Optum family, seeks an experienced Hospice Registered Nurse Case Manager to oversee delivery of the plan of care for each hospice patient. You will collaborate with professionals to implement treatment strategies that promote physical, psychosocial, emotional, and spiritual well-being.

Responsibilities include initial eligibility assessments, care plan initiation, and ongoing documentation, with a focus on compassionate, patient-centered

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies patient's/family's physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG members in daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes regarding patient condition.
• Function in diverse home settings regardless of age, race, creed, color, sex, disability, or financial condition.
