Job summary

Heart of Hospice, a part of LHC Group, seeks a Registered Nurse Case Manager to deliver and evaluate the Plan of Care for hospice patients. You will collaborate with professionals to implement treatment strategies spanning physical, psychosocial, emotional, and spiritual well-being.

Responsibilities include initial eligibility evaluation, coordinating the care plan, and documenting progress while ensuring quality and productivity in a compassionate home-care setting.

Qualifications

• Current and unrestricted RN license in the state of practice.
• Valid driver's license, vehicle insurance, and reliable transportation or access to public transit.
• Minimum 1 year of full-time RN experience; 3 years as an LPN may substitute for the RN experience.

Responsibilities

• Perform the initial nursing evaluation within 48 hours of referral.
• Identify patient/family physical, psychosocial, and environmental needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician and other IDG members daily.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient's condition.
• Deliver care in diverse home settings regardless of age, race, creed, color, sex, or disability.
