Job summary

UnitedHealth Group, through the Optum family, seeks an RN Case Manager to oversee the Plan of Care for hospice patients in Washington. You will collaborate with clinicians to promote physical, psychosocial, emotional and spiritual well-being, even in challenging home settings.

Responsibilities include timely initial evaluation, ongoing reassessment, care coordination, and thorough documentation. A supportive team environment and development opportunities are provided.

Qualifications

• Current and unrestricted RN licensure in the state of practice (Washington).
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Makes initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with the IDG.
• Documents problems, goals, interventions and responses to hospice care.
• Collaborates with patient/family, attending physician and other IDG members daily.
• Instructs and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes; works in diverse home situations.
