Job summary

UnitedHealth Group seeks an experienced Registered Nurse Case Manager for hospice services. You will assume full nursing responsibility for plans of care, develop strategies with other professionals, and ensure quality, compassionate care for patients facing end-of-life needs.

Responsibilities include initial eligibility assessment within 48 hours of referral, ongoing reassessments, and daily collaboration with the care team.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• 1+ years of clinical experience preferred; CPR certification required or achievable within 90 days.

Responsibilities

• Make initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Identify patient/family needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care and document patient/family response.
• Collaborate with the patient/family, attending physician, and IDG to provide care daily.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition.
