Job summary

Optum's At Home Hospice division seeks a Hospice Registered Nurse Case Manager to lead the Plan of Care for each patient in a home-based setting. You will evaluate performance, collaborate with professionals, and adjust treatments to promote holistic well-being for patients and families.

Qualifications

• Current and unrestricted RN license in the state of practice.
• Current driver's license, valid vehicle insurance, and access to dependable transportation.
• CPR certification or ability to obtain within 90 days of hire (preferred).

Responsibilities

• Make initial nursing evaluation within 48 hours of referral to determine eligibility.
• Identify physical, psychosocial, and environmental needs; reassess every 15 days.
• Initiate and coordinate the plan of care; document goals, interventions, and responses.
• Collaborate with patient/family, attending physician and IDG for daily care.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition.
• Provide care in home settings regardless of age, race, creed, or condition.
