Job summary

UP Health System Hospice, part of LHC Group within Optum, is seeking a Registered Nurse Case Manager to deliver and coordinate the hospice plan of care for patients and families served. The role requires compassionate care across physical, psychosocial, and spiritual dimensions and collaboration with the IDG.

You will perform initial eligibility evaluations within 48 hours of referral, coordinate care with the team, and document patient progress while maintaining compliance and high-quality care

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG to provide daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes regarding patient condition.
