Job summary

Four Seasons is seeking a Support RN to uphold our hospice mission in the West Region and surrounding areas. This role reports to the Clinical Team Leader and is structured as hourly, non-exempt, with a 40-hour work week and a Monday–Friday 8am–4:30pm shift.

The RN will perform admissions and patient visits as needed, conduct comprehensive patient assessments, and deliver direct nursing care aligned with the care plan and regulatory requirements.

Qualifications

• Provide nursing care to hospice patients.
• Perform comprehensive patient assessments.
• Plan, initiate, deliver, and evaluate nursing acts.

Responsibilities

• Practice within licensed scope and comply with regulations.
• Assess physical and mental health of patients.
• Record nursing assessments and report results.
• Coordinate care with other health professionals.
