Job summary

Four Seasons seeks an Admissions RN to act as an ambassador for our hospice mission, managing 2–4 admissions daily and occasionally conducting patient visits. You will assess patients, educate families, and coordinate with referring sources to support high‑quality care.

You will collaborate with the healthcare team, follow regulatory requirements, and ensure accurate documentation while maintaining compassionate patient-centered care in a fast-paced environment.

Qualifications

• Must be willing to care for patients with infectious disease processes
• Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
• Knowledge of HIPAA privacy rules and regulations
• Knowledge of electronic medical records systems
• Demonstrate excellent written and oral communication skills
• Ability to work under pressure to meet deadlines
• Ability to take initiative and utilize innovative techniques and ingenuity
• Ability to interact with various departments throughout the organization
• Ability to work flexible hours, if needed
• Ability to participate as a team player to coordinate projects
• Ability to plan, prioritize and coordinate multiple projects
• Demonstrate skill/expertise in pain and symptom management
• Demonstrate ability to effectively collaborate and facilitate the work of a group
• Demonstrate understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation

Responsibilities

• Perform admissions daily and patient visits as needed
• Provide direct nursing care when required
• Conduct comprehensive patient assessments to identify needs
• Educate patients and families about hospice services
• Develop and sustain partnerships with referral sources in the community
• Coordinate with health care providers to plan patient-centered care
• Obtain consents and ensure safety of nursing care
• Supervise and educate staff performing nursing tasks
• Maintain and report nursing plans and patient responses
• Ensure compliance with regulatory standards and documentation
