Job summary

Four Seasons is seeking a Hospital Liaison RN to serve as a bridge between contracted hospitals and our hospice team. You will perform inpatient visits, provide education to providers, and guide patients and families through hospice options and admission processes.

The role involves developing care plans with the medical team, coordinating with discharge planners, and expanding partnerships to support Four Seasons’ mission. Strong nursing assessment and communication skills are essential.

Qualifications

• Minimum 2 years clinical experience preferred
• 1 year hospice experience preferred
• Demonstrated superior customer service skills
• Pain and symptom management experience
• Basic computer skills with word processing ability
• Ability to collaborate with interdisciplinary team
• Ability to respond sensitively to patients and families and staff

Responsibilities

• Act as liaison between contracted hospitals and the hospice team
• Perform pre-admission evaluations for hospice suitability
• Educate patient/family about hospice services
• Develop care plans with patient goals and medical team
• Coordinate discharge planning and community partnerships
• Build relationships with hospital discharge planners and census development teams
• Ensure nursing care aligns with license requirements and Four Seasons policies
