Job summary

Four Seasons is seeking an RN Case Manager to coordinate nursing care for a caseload in Macon County and surrounding areas. The role requires direct nursing care, supervision of LPNs and aides, and collaboration with the care team to deliver patient-centered plans.

We offer a salaried, exempt position with regular hours Monday–Friday, 8:00 AM–4:30 PM, and rotating emergency weekend/holiday calls. Strong clinical experience and organizational skills are essential.

Qualifications

• Two years of clinical experience preferred.
• Valid NC RN license is required.
• Ability to coordinate care and supervise team members.

Responsibilities

• Coordinate the nursing plan of care for an assigned caseload.
• Provide direct nursing care and supervise LPNs and Hospice Aides.
• Assess patient health and document nursing assessments.
• Plan and evaluate nursing interventions and outcomes.
• Teach and supervise staff in treatment regimens and procedures.
• Collaborate with other providers to ensure patient-centered plans.
