Job summary

Four Seasons is seeking a Hospital Liaison RN to act as the bridge between contracted hospitals and our hospice team. You will visit patients in facilities, provide education to providers, and support the admission process with patient-centered care plans aligned to Four Seasons policies.

Responsibilities include pre-admission evaluations, coordinating with the medical team, and developing comprehensive care plans.

Qualifications

• Minimum RN license in NC and hospice experience preferred.
• Strong nursing assessment and care planning skills.
• Ability to educate patients and families on hospice services.

Responsibilities

• Serve as liaison between hospitals and hospice team.
• Conduct inpatient visits and provide in-service education.
• Develop patient-centered care plans with the medical team.
• Obtain informed consents and coordinate admissions.
• Coordinate discharge planning and community partnerships.
