Chapters Health System is seeking a Per Diem Registered Nurse in the Orlando area to provide compassionate hospice care to patients and families. You will assess needs, coordinate care with the IDG, and deliver palliative support in a variety of settings.

The role requires an active RN license in Florida, BLS certification, and experience in hospice or hospital settings. Valid driver’s license and reliable transportation are needed to meet visit schedules.

Qualifikationen

• Current RN license in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one year nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems.

Aufgaben

• Provide direct care to patients and families as part of the Interdisciplinary Group (IDG).
• Coordinate Plan of Care reflecting patient needs.
• Educate patient and family about disease, palliative care, and safety practices.
• Report changes in patient condition to IDG or other health professionals.
• Participate in evaluation of hospice referrals/admissions for level of care.
• Attend patient deaths and prepare body for transport.
