Job summary

Chapters Health System seeks an Admission RN to assess patients in their residence or hospitals and initiate care within our Hospice services. This role emphasizes seamless transition to Care Navigation and serving as an ambassador of the organization.

Requirements include RN license in the state, 1 year nursing experience (hospice/hospital preferred), EMR experience, strong assessment skills, English proficiency (Spanish preferred), and a valid driver’s license with reliable transportation.

Qualifications

• Must hold RN license in the state where working.
• Minimum of 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; background in ED or ICU preferred.
• English communication skills required; Spanish is a plus.
• Valid driver’s license and automobile insurance as per policy.
• Reliable transportation and ability to work in a team.

Responsibilities

• Complete clinical admissions for services and explain offerings to patients/families.
• Identify admission barriers and provide responses; educate on admission requirements.
• Initiate Plan of Care with physicians and document per policy.
• Coordinate care with Care Navigation and referral sources; SBAR communications.
• Manage admissions with multiple priorities and prepare for next assignment.
• Support after-hours staff and ensure timely discharge planning.
