Job summary

Chapters Health System is seeking an RN Care Coordinator to assess patients and families, coordinate the plan of care with the IDG, and provide palliative and supportive care in patients’ homes and facilities across Florida.

The role requires an active RN license in Florida, at least 1 year of nursing experience (hospice/hospital preferred), and strong communication. Proficiency with EMR/EHR systems and MS Office is expected; bilingual Spanish is a plus.

Qualifications

• Current RN license in the state of employment.
• Minimum of 1 year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• English communication skills required; bilingual Spanish preferred.

Responsibilities

• Make after-hours home visits to patients and provide compassionate customer service to patients and families.
• Assess patient/family needs and identify nursing interventions by phone or during visits.
• Provide direct care as part of the Interdisciplinary Group (IDG) and coordinate care.
• Educate patients, families and caregivers about disease process, palliative care, and safety practices.
• Report changes in patient condition to IDG; ensure timely documentation and follow-up.
