Job summary

Gentiva Hospice is seeking an RN Case Manager to lead comprehensive home-based end-of-life care. You will assess patients, implement and document plans of care, and coordinate with a multidisciplinary team to ensure compassionate, high-quality hospice services.

Strong communication and clinical documentation skills are essential, with a focus on patient and family education. The role requires an active RN license, 1+ year of RN experience in hospice or related fields, and the ability to travel

Qualifications

• Active RN license in state of employment or eligible to obtain
• Minimum of 1 year RN experience in acute care, hospice, home health or related setting
• Hospice or palliative care experience preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans and visit frequency.
• Coordinate care with interdisciplinary team and physicians.
• Educate patients, families and caregivers on disease, end-of-life care and medications.
• Document timely clinical records and maintain infection control compliance.
• Support quality improvement and scheduling initiatives and transitions in care.
