Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead comprehensive home-based care for patients with life-limiting illnesses. You will assess, plan, and implement personalized care, coordinating with an interdisciplinary team to ensure comfort and dignity for patients and families.

Responsibilities include education for families, symptom management, and documentation while maintaining compliance and high-quality care in home or community settings.

Qualifications

• RN license in state of employment.
• CPR Certification.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, with education for patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency.
