Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to lead patient care in homes and care facilities. You will assess, plan, implement, and document personalized hospice services, coordinating with an interdisciplinary team to ensure compassionate end‑of‑life care for patients and families.

Requirements include an active RN license, CPR, and at least 1 year in hospice, home health, or acute care. Commitment to quality, compliance, and caregiver education is essential.

Qualifications

• RN with active license in the state of employment.
• Minimum 1 year RN experience in acute care, hospice, or home health.
• Hospice or palliative care experience preferred.
• CPR certification required.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver and document skilled nursing care per Plan of Care.
• Coordinate care with interdisciplinary team (physicians, LPNs, CNAs, social workers, chaplains).
• Provide education and bereavement guidance to patients and families.
• Maintain timely clinical records and adhere to infection control standards.
• Participate in on‑call and patient support activities as applicable.
