Job summary

Gentiva Hospice is seeking a dedicated RN Case Manager to lead in-home hospice care, coordinating compassionate, evidence-based nursing services for patients and families in their homes or community settings.

You will assess, plan, educate, and support patients, families, and the care team while maintaining clinical documentation and compliance with regulatory standards. This role emphasizes empathy, advocacy, and collaborative care across disciplines.

Qualifications

• Current RN license in the state of employment.
• Minimum of 1 year RN experience in acute care, oncology, long-term care, hospice, or home health.
• Hospice or palliative care experience preferred.
• CPR certification.
• Current automobile insurance and valid driver's license.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care in patients' homes, assisted living, or other settings.
• Perform initial and ongoing assessments to monitor patient condition.
• Collaborate with an interdisciplinary team to refine care plans.
• Provide emotional, physical, and spiritual support to patients and families; educate and support caregivers.
• Communicate updates to the care team and ensure timely interventions.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives to ensure compassionate, efficient care.
• Participate in on-call and patient support activities as applicable.
