Gentiva Hospice is seeking a dedicated Registered Nurse Case Manager to lead and coordinate in-home hospice care. You will assess patients, implement plans of care, and collaborate with an interdisciplinary team to ensure compassionate, comprehensive support for patients and their families.

Responsibilities include education for families, symptom management, and timely documentation. This role requires a valid RN license, CPR certification, and at least one year of RN experience in hospice, home

Qualifications

• Current RN license in the state of employment.
• CPR Certification.
• 1 year RN experience in acute care, hospice, home health or related field.

Responsabilités

• Perform comprehensive patient assessments and reassessments in home settings.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care and visit schedules.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and caregivers on disease, care needs, and bereavement support.
• Document timely clinical records and maintain infection control standards.
• Participate in on-call and patient support activities.
