Job summary

Gentiva Hospice is seeking a skilled Hospice RN/Case Manager to lead patient care in home settings across the community. You will assess patients, implement plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate end-of-life support.

Your role includes educating families and providing bereavement guidance. Requirements include an active RN license and a minimum of 1 year of nursing experience; hospice experience is preferred.

Qualifications

• RN license required for the state of employment.
• Minimum 1 year experience as an RN in healthcare settings.
• Hospice or palliative care experience preferred.

Responsibilities

• Be the primary contact for patients and families throughout hospice care.
• Deliver and document skilled nursing care per each patient’s plan of care.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to refine care plans.
• Provide education and bereavement support to patients and caregivers.
• Teach caregivers and communicate important updates to the care team.
• Maintain accurate clinical documentation and ensure compliance.
• Participate in on-call services and patient support activities.
