Graham Healthcare Group in Staunton, IL is hiring a Registered Nurse (RN) for Hospice to provide end-of-life care in patients' homes. You will join an interdisciplinary team delivering compassionate, patient-centered care with autonomy and opportunities for growth.

The role requires an active RN license in your state, at least two years of RN experience (Hospice preferred), a valid driver's license, and willingness to drive ~20% of the time.

Responsibilities

• Maintain accurate EMR documentation and ensure compliance.
• Work independently, but collaborate as needed with the team.
• Travel to patient homes within the assigned territory.

• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
