Residential Home Health and Hospice in Springfield, IL, is seeking a Registered Nurse for a hospice role delivering end-of-life care in patients' homes. A $10,000 first-year success bonus is available to reward milestone achievements in the first year.

Qualifications

• Active Registered Nurse license in the state you work.
• Two years of RN experience with hospice experience preferred.
• Valid driver’s license and ability to travel ~20% of work time.

Responsibilities

• Perform comprehensive patient assessments and symptom/pain management; plan end-of-life care.
• Educate and support patients and families through the end-of-life journey; ensure informed decisions.

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work

• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
