Job summary

Graham Healthcare Group in Wheeling, IL is seeking an experienced Registered Nurse (RN) to provide end-of-life care in patients' homes within the Wheeling area. This hospice role emphasizes compassionate patient-centered care and collaboration with an interdisciplinary team.

Eligible for a $10,000 first-year bonus; strong autonomy, growth opportunities, and support from a dedicated field education team. Travel required to patient homes; IL RN license and 2 years' RN experience preferred.

Qualifications

• Valid driver's license and ability to travel ~20% of workday.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life

• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning

• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work

• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred

• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
