Job summary

Graham Healthcare Group in Oak Brook, IL seeks an experienced RN to provide end-of-life hospice care in patients' homes. This role includes a $10,000 first-year success bonus and meaningful patient relationships.

Responsibilities: comprehensive assessments, symptom management, family education, EMR documentation, and travel within the territory. Illinois RN license required; hospice experience preferred. Team-oriented, supportive culture.

• Valid driver’s license for travel to patient homes.

• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning

• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work

• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
