Job summary

Residential Home Health and Hospice in Colonial Park, PA is hiring a Registered Nurse (RN) to provide end-of-life care in patients' homes. This role offers a $10,000 first year success bonus to reward milestones during the first year.

You will deliver compassionate hospice care, perform patient assessments, manage symptoms, educate families, and document in the EMR. Travel to patient homes is required; field education and leadership support will help you grow professionally.

Qualifications

• Active RN license in the state of employment.
• Two years of RN experience, hospice preferred.
• Valid driver's license and ability to drive ~20% of the day.

Responsibilities

• Deliver compassionate, patient-centered hospice care at patients' homes.
• Perform assessments, symptom and pain management, and end-of-life care planning.
• Educate and support patients and families through the end-of-life journey.
• Maintain compliant EMR documentation.
• Travel to patient homes within assigned territory.

• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN Qualifications

• Active Registered Nurse, RN license in the state you work
• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
• Current driver\’s license and ability to spend ~20% of your day driving to/from patient locations
