Job summary

Residential Home Health and Hospice in the Chicago, IL area is seeking a dedicated Registered Nurse (RN) to provide end-of-life care to patients in their homes. This role offers a $10,000 first-year success bonus to reward milestone achievements during your first year.

In this hospice position, you will deliver compassionate, patient-centered care, assess symptoms, manage pain, and educate families on the journey with dignity and support.

Qualifications

• Active RN license in state of work.
• Two years of RN experience; hospice experience preferred.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life.
• Perform comprehensive patient assessments, symptom and pain management, and individualized end-of-life care planning.
• Educate and support patients and families through the end-of-life journey, ensuring they feel informed, prepared, and deeply cared for.
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
• Travel to patient homes within assigned territory.

RN Qualifications

• Active Registered Nurse, RN license in the state you work

• Two years of RN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
