Job summary

Lower Cape Fear LifeCare is seeking a Hospice Registered Nurse to provide in-home and facility-based end-of-life care in the Wilmington, NC area. You will deliver compassionate nursing services, educate caregivers, and coordinate with an interdisciplinary team to ensure comfort and quality of life for patients and families.

The role requires a valid NC nursing license, at least two years of experience (preferably in home care or hospice), and the ability to drive to patient sites.

Qualifications

• Graduate of an approved school of nursing (AD, Diploma or BSN).
• Current license to practice professional nursing in the State of North Carolina.
• Two years of nursing experience preferred, home care or hospice experience valued.
• Ability to document and communicate clearly with patients, families, and staff.
• Ability to lift up to 45 pounds and drive to patient sites.

Responsibilities

• Provide patient care in patients' homes and facilities.
• Train caregivers in wound care, medications, and activities of daily living.
• Document patient information and coordinate care with the IDT.
• Drive between patient visits using personal vehicle with mileage reimbursed.
• Participate in team meetings to discuss patient eligibility and plans of care.
