Job summary

Well Care Health in North Carolina is seeking a compassionate Hospice Registered Nurse Case Manager to deliver expert end-of-life care and support to patients and families. You will assess needs, develop individualized care plans with the hospice interdisciplinary team, educate families about hospice processes, and coordinate services to ensure comfort and quality of life.

This role requires NC RN license and CPR, with at least one year of clinical experience (hospice preferred); some on-call

Qualifications

• Graduate of NLN accredited school of nursing and NC RN license.
• Current American Heart Association CPR certification.
• Minimum of one year clinical experience; hospice experience preferred.
• Strong communication and interdisciplinary teamwork skills.
• Ability to work independently with strong organization.
• Physical ability to perform duties; some on-call may be required.

Responsibilities

• Complete initial, comprehensive, and ongoing assessment of patient and family needs and provide nursing services based on plan of care.
• Develop an individualized plan of care with the hospice interdisciplinary team and family, incorporating palliative interventions.
• Communicate with attending and hospice physicians regarding needs and treatment responses.
• Collaborate with IDG and family to provide holistic care, including bereavement support.
• Demonstrate mission, vision, and values of the agency daily.
