Lower Cape Fear LifeCare is seeking a compassionate Hospice Registered Nurse to join our team in Lumberton, NC. The role involves full-time, home-based patient visits, coordination with an interdisciplinary team, and guidance to family caregivers.

You will travel to patients' homes, document care plans, and ensure comfort during end-of-life care. A valid RN license and admission to NC board are required, with 2 years of relevant experience preferred.

Qualifications

• Graduate of an approved school of nursing with license to practice as RN.
• Two years experience in Home Care, Hospice, or related oncology preferred.
• Ability to document and communicate clearly with patients, families, physicians, and staff.
• Must be able to lift and move patients and manage physical demands of home visits.
• Strong English language literacy and interpersonal skills are essential.
• Willingness to drive your own vehicle and travel between patient homes.

Responsabilités

• Provide end-of-life care at patients' homes, assisted living, or SNFs.
• Coordinate with interdisciplinary team to plan and implement care.
• Train caregivers on wound care, medications, and personal care.
• Document patient status, orders, and care plans in real time.
• Manage travel between visits and reimburse mileage per policy.
• Respond to patient needs and emergencies with flexible scheduling.
