Job summary

Lower Cape Fear LifeCare is seeking an experienced Nurse Practitioner to join our hospice and palliative care team. This full-time role focuses on patient-centered care across homes, clinics, and community settings with telehealth support.

You will educate caregivers, manage end-of-life discussions, and coordinate with a multidisciplinary team to ensure high-quality care. A Master's in Nursing and NP certification are required; five years medical experience with two as an NP is expected.

Qualifications

• Master's Degree in Nursing required.
• Current ANCC/ANNP certification preferred.
• Minimum five years of general medical experience in inpatient, outpatient, or nursing home settings; two years as a nurse practitioner required.

Responsibilities

• See patients in homes, clinics, assisted living facilities, and other care venues; includes telehealth support.
• Educate caregivers on disease education, advance care planning, medications, and end-of-life expectations.
• Travel between patient visits using personal vehicle with mileage reimbursement.
• Coordinate care with community medical providers and IDT; updates and new orders are obtained during and between visits.
• Self-scheduling based on patient needs and acuity.
