Job summary

Lower Cape Fear LifeCare is seeking a Hospice Registered Nurse to provide home-based care across our service area. We welcome dedicated RNs to support patients in personal residences, assisted living facilities, and skilled nursing facilities, delivering compassionate end-of-life care.

Responsibilities include patient assessment, caregiver training, and real-time documentation, with mileage reimbursement for travel.

Qualifications

• Graduate of an approved nursing program (AD, Diploma, or BSN).
• Current license to practice professional nursing in South Carolina.

Responsibilities

• Care for patients at home or in facilities, accommodating diverse backgrounds.
• Train caregivers in wound care, medication administration, personal care, and end-of-life expectations.
• Travel between patients using your own vehicle with mileage reimbursement.
• Coordinate care between community providers, hospice team and other disciplines between visits.
• Real-time documentation of patient care and orders; maintain up-to-date charts.
• Be part of an Interdisciplinary Team that meets biweekly to discuss patient eligibility.
