Job summary

Enhabit Home Health & Hospice seeks a skilled professional to manage relationships with referral sources and educate partners on service offerings. The role focuses on growing referrals, ensuring smooth post-acute transitions, and improving patient outcomes.

The candidate should have healthcare business development experience, strong communication skills, and a valid driver’s license. In-office attendance is essential to achieve objectives.

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field or graduate of an approved school of nursing, therapy or social work.
• Highly preferred graduate of an approved school of nursing, therapy or social work.
• Minimum two years of experience in healthcare business development, community relations, marketing, or sales.
• Experience with federal, state, and local laws governing Medicare certified home health/hospice operations.
• Valid state driver license and automobile liability insurance.

Responsibilities

• Serve as the primary relationship manager between Enhabit and referral sources (physicians, hospitals, health systems, long-term care and senior living communities).
• Educate referral sources about Enhabit services and program requirements to drive referral growth.
• Support transitions to post-acute care and aim to improve patient outcomes and satisfaction.
