Job summary

Enhabit Home Health & Hospice is seeking a dynamic relationship manager to cultivate referrals from physicians, hospitals, and senior living communities. You will educate partners on services, navigate post-acute transitions, and support high-quality patient outcomes.

Qualifications

• Bachelor's degree or equivalent in healthcare/related field; or graduate of nursing, therapy, or social work.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Valid state driver's license and automobile liability insurance.
• Strong interpersonal, communication and presentation skills.
• Clinical licensure if applicable; preferred roles may require it.

Responsibilities

• Serve as primary relationship manager with referral sources to drive growth.
• Educate referral sources on services and program requirements to improve outcomes.
• Support post-acute care transitions and navigation to ensure quality care.
• Promote exceptional patient and referral source satisfaction through effective outreach.
