Responsibilities

• Serves as primary relationship manager between Enhabit and referral sources (physicians, hospitals, health systems, senior living communities, agencies).
• Promotes awareness of services and program requirements to referral sources, patients and families to improve post-acute care transitions.
• Supports referral growth and satisfaction through education and relationship management.
