Job summary

Dignity Care Hospice in Indianapolis, IN is seeking a Hospice Account Executive to cultivate relationships with referral sources and guide patients and families through the hospice referral and admission journey. You will educate providers, hospitals, clinics, senior living communities, and families about hospice services, coordinating referrals and supporting the care team to ensure timely access.

You will leverage CRM tools to track referrals and work closely with the IDT to align capacity

Qualifications

• Experience in B2B or B2C healthcare sales is preferred.
• Proven ability to build and maintain professional relationships with referral partners.
• Strong communication skills with the ability to educate diverse audiences.

Responsibilities

• Build and maintain relationships with physicians, case managers, hospital staff, and senior living communities to educate them on hospice services and referrals.
• Coordinate timely hospice referrals by gathering clinical information and supporting eligibility assessments.
• Conduct community education and outreach to raise awareness of hospice philosophy and benefits.
• Support families and patients through the referral and admission process.
• Collaborate with the Interdisciplinary Team (IDT) to track capacity and referral activity.
