Job summary

CommuniCare Health's Dignity Care Hospice is seeking a Hospice Account Executive in Independence, OH to connect patient needs with hospice services and coordinate care transitions from acute settings. You will build relationships with case managers and hospital staff, promote hospice benefits, and ensure compliant patient referrals while representing our mission of compassionate, patient-centered care.

Two years sales/marketing experience required; valid driver's license; a passion for serving

Qualifications

• Two years of experience in sales and marketing.
• Strong communication and interpersonal skills.
• Ability to build and maintain professional relationships with hospital staff and case managers.

Responsibilities

• Communicate with interdisciplinary team daily to coordinate patient care and bed availability.
• Manage electronic referral system and develop relationships with discharge planners and hospital staff.
• Complete clinical reviews and coordinate transitions to assigned centers.
• Remain visible as a hospital liaison and promote hospice services.
• Ensure compliance with HIPAA and patient confidentiality.
