Job summary

Qualifications

• Two year college (Associate’s Degree)
• 1 – 3 years (minimum of 2 yrs exp)
• Minimum Associates degree. Preference given to candidate with bachelor’s degree in business, Marketing or Health Sciences related field.
• Minimum of two (2) years’ sales experience strongly preferred.
• Familiarity with general health care concepts and terminology. Knowledge/experience of hospice strongly preferred.

Responsibilities

• The Hospice Liaison serves as the primary community relationship and business development representative for the agency, acting as a trusted resource for healthcare providers, patients, families, and community partners.
• Developing, maintaining, and expanding referral relationships within an assigned territory to increase awareness, understanding, and utilization of hospice services.
• Collaborates with hospitals, physician practices, skilled nursing facilities, assisted living communities, rehabilitation centers, case managers, discharge planners, social workers, and other referral sources to identify patients who may benefit from hospice care.
