Job summary

The Care Team Hospice is seeking a driven, relationship-focused Community Care Liaison (CCL) to grow our presence in the community. This high-impact role builds connections with physicians, hospitals, and referral sources to increase awareness, trust, and utilization of hospice services.

You will educate providers, patients, and families about the value of hospice, aim to grow census through partnerships, and represent the hospice with empathy and professionalism.

Qualifications

• Bachelor’s degree or equivalent.
• 1–2 years healthcare outside sales experience.
• 1–2 years Hospice or Home Health sales preferred.

Responsibilities

• Build and expand relationships with physicians, healthcare partners, and key decision-makers to drive referral volume and market growth.
• Educate the community and share the value of comfort-focused, patient-centered hospice care.
• Achieve census and admission targets by positioning hospice as a preferred provider in the community.
• Engage regularly with healthcare providers and referral sources to create strong, collaborative partnerships focused on patient well-being.
• Lead hospice discussions with patients and families, clearly communicating services with professionalism and empathy.
