Job summary

Affinity Hospice is seeking a Clinical Liaison to cultivate relationships with hospitals, physicians, and discharge planners to grow hospice referrals and engage the community. The role blends marketing strategy with clinical support to drive census growth and ensure high-quality patient care.

Responsibilities include coordinating outreach programs, educating patients and families, and collaborating with leadership to align goals and initiatives. This is a full-time, on-site role in Atlanta.

Qualifications

• Associate/bachelor's degree in nursing or related field preferred.
• Minimum of two years of relevant experience required.
• Hospital and/or long-term care clinical experience preferred.
• Experience in healthcare marketing preferred.
• Active and unencumbered RN employment required.
• Valid driver's license with access to insured vehicle.

Responsibilities

• Build and maintain relationships with hospitals, physicians, discharge planners, and other referral sources.
• Develop and implement structured outreach to identify new referral sources.
• Drive census growth and community awareness through marketing and events.
• Support operational and clinical needs across the referral/admission process.
• Educate patients and families about hospice services and benefits.
