Job summary

Responsibilities

• Develop, maintain and expand referral relationships within an assigned territory.
• Educate providers, patients and families about hospice eligibility and benefits.
• Collaborate with hospitals, physician practices and care facilities to identify potential patients.
• Facilitate timely referrals and admissions; support census growth and customer satisfaction.
