Job summary

Kindful Health is seeking a Hospice Sales Liaison to cultivate new business, nurture referrals, and coordinate with care teams to ensure smooth admission to hospice services. The role involves communicating with families, physicians, discharge planners, and external vendors within a defined territory.

The position emphasizes adherence to a sales process, maintenance of referral sources in CRM, and development of collaborative relationships to meet short- and long-term admission goals.

Qualifications

• Bachelor's degree preferred and at least two years of Hospice sales experience.
• Generates qualified referrals to achieve hospice admissions and meet target goals.
• Maintains up-to-date records of referral sources within the CRM.
• Assists care center in obtaining documentation to complete referral-to-admission.
• Evaluates patient referrals for appropriateness of admission to hospice.
• Provides consultation to referrers, patients and families about hospice care options.
• Communicates patient needs to the care team and coordinates resources.

Responsibilities

• Develops and implements a sales/marketing plan for the assigned region.
• Establishes relationships with physicians, hospitals and facilities to drive referrals.
• Acts as liaison during transition from discharge to hospice initiation.
• Maintains ongoing collaboration with referral sources and institutional personnel.
• Prepares business plans and maintains target lists; prioritizes accounts.
• Responds to customer concerns in a timely manner.
