Job summary

Hartford HealthCare at Home is seeking a QAPI Specialist to drive clinical quality and safety through data-driven improvement. You will compile trends and reports across patient care, safety, risk management, infection control, outcomes and customer satisfaction, and lead development of Performance Improvement Plans.

The role requires 3 years of certified home health experience, knowledge of PDGM and CAHPS, CT licensure, strong communication, and willingness to travel to branches nationwide to

Qualifications

3 years of certified home health experience.
Knowledge of Value Based Purchasing, PDGM, CAHPS-preferred.
CT licensure in field of specialty.
Ability to travel to branches as needed.

Responsibilities

Assist with agency efforts toward QAPI and data-driven quality improvement.
Compile trends and reports on patient care, safety, risk management, infection control, outcomes and satisfaction.
Develop Performance Improvement Plans and monitor audits.
Ensure regulatory compliance and timely data submissions to payors and agencies.
Provide education and feedback to staff.
Facilitate PI teams using quality improvement methodology.
Promote diversity, health literacy, and language inclusivity.
